Student Registration Packet for Parents

NEW REGISTRATIONS:
For students NEW to Miami-Dade County Public Schools (i.e., students transferring to Miami Killian Senior High School from private schools, or out-of-county, state, or country schools) AGE AND LEGAL NAME VERIFICATION IS REQUIRED. ONE of the following must be presented:
1. Duly attested original birth certificate or birth card – Must be original; hospital certificate not acceptable
2. Duly attested Certificate of Baptism with a parent affidavit
3. Insurance policy on the child’s life in force for two years
4. Bona fide religious record with parent affidavit
5. Passport or Certificate of Arrival in the U.S. showing age of child
6. Transcript of school records of at least four years prior, stating date of birth
7. Affidavit of age signed by parent and Certificate of Age signed by public health officer
VERIFICATION OF ADDRESS IS REQUIRED. TWO of the following must be presented:
1. Broker’s or Attorney’s statement of parents’ purchase of residence OR properly executed lease agreement
2. Current Homestead Exemption Card
3. Electric deposit receipt or electric bill, showing name and service address
4. Miami-Dade County Public Schools Statement of Bona Fide Residence (FM-7444)
HEALTH REQUIREMENTS ARE REQUIRED. BOTH of the following must be presented:
1. Student Health Examination – DH 3040 yellow form health examination performed within one year prior to enrollment
AND
2. Florida Certificate of Immunization – DH 680 blue card from a private doctor or local health provider
SCHOOL RECORDS ARE REQUIRED. Official transcripts are required from the prior school of attendance. If the official transcript is not available, any record of prior school attendance can be used temporarily.
Note the following regarding prior school records:
1. Records are used for grade placement and verification of credits earned
2. Interpretation of foreign records, at no cost, is available from the Federal and State Compliance Office. Registration Office staff will assist with this process, as needed.
THE FOLLOWING COMPLETED FORMS ARE REQUIRED. Please complete all forms that apply:
1. Form 1867 Permission for Release of Records (required)
2. Form 2733 Emergency Student Data (required)
3. Form 6008 For New Student without School Record (if applicable)
4. Form 6479 Directory Information (required)
STUDENT TRANSFERS:
For students TRANSFERRING to Miami Killian Senior High School from another Miami-Dade County Public School (including charter school transfers), A WITHDRAWAL SLIP FROM THE PRIOR SCHOOL IS REQUIRED.
AND
VERIFICATION OF ADDRESS IS REQUIRED. TWO of the following must be presented:
1. Broker’s or Attorney’s statement of parents’ purchase of residence OR properly executed lease agreement
2. Current Homestead Exemption Card
3. Electronic deposit receipt or electric bill, showing name and service address
4. Miami-Dade County Public Schools Statement of Bona Fide Residence (FM-7444)
STUDENT WITHDRAWALS:
For students WITHDRAWING from Miami Killian Senior High School, VERIFICATION OF ADDRESS IS REQUIRED. TWO of the following must be presented:
1. Broker’s or Attorney’s statement of parents’ purchase of residence OR properly executed lease agreement
2. Current Homestead Exemption Card
3. Electric deposit receipt or electric bill, showing name and service address
4. Miami-Dade County Public Schools Statement of Bona Fide Residence (FM-7444)
